Good question! For print book, you will get a Createspace print ready PDF file. For kindle formatting, you will .MOBI files.
Do you format cover?
Yes I can do cover from scratch. and I can convert your kindle cover to print cover as well.
Do you do children/picture/illustrated book?
Yes I can do children book (Bleed book) Check my other gig which is specially for Children book.
What type of images should be used in a print book? 300dpi!
We need 300 for print books.
Is a Table of Contents Included? What about headers and page numbers?

NO! One order/gig is for either of kindle OR Createspace formatting and not for both. If you need both, change your quantity to 2 while ordering
