Do you write the survey questions?
You provide the final questions, choices and logic. I convert them into the form. Research design, statistical advice and writing the questionnaire are outside this gig.
What is an XLSForm?
An editable Excel workbook that describes your questions, choices and rules. KoboToolbox imports it to build the survey. You receive the source so it can be maintained later.
Do we need a call?
No. We agree the scope, review questions and deliver files through Fiverr messages.
What counts as a question or skip rule?
One response field is one question. One skip rule controls one target question or group with at most two simple conditions. Each numeric range check sets one field’s minimum and maximum. Complex logic and repeats are outside these packages.
How do you check the form?
I check the workbook against your source, test each skip rule shown and hidden, and check invalid/boundary values. Your test record names the environment and results. We agree a private Kobo import/preview route before ordering and complete that check before delivery.
Are there limits on question length and answer choices?
Each choice question allows up to 10 options. Total options: 75 Basic, 150 Standard, 250 Premium. Source text is limited to 100 words per question, including choices. Send longer questions, grids or larger lists before ordering for a scope review.
Do you support ODK, KoboCollect or SurveyCTO?
This gig targets KoboToolbox web forms. ODK, KoboCollect and SurveyCTO share some XLSForm features, but compatibility, device and offline testing require separate agreement and completed tests. They are outside the initial packages.
Do you use AI?
I may use AI to draft structure and logic; I review the file and test the agreed rules. Tell me before ordering about any AI restrictions. I will not send confidential content to external AI without your approval.
What does the revision include?
One consolidated round covers the agreed source and layout. New questions or changed rules cost extra. Errors I introduce against agreed requirements are fixed without using your revision when reported within seven calendar days of delivery.
Do you upload or collect responses?
You control the private project, publish the form and collect responses. I deliver the XLSX and instructions, with the agreed private preview check. Hosting, live deployment and handling respondent data are not included.
