I create dynamic forms using multiple different options in laravel.

I already have code with few options to create and share dynamic forms. and i make any customization as your requirements.

Let's i describe here what i already have developed.

Fields List

• Text Input - required options .
• Radio Input - multiple options , description
• Checkbox - multiple options ,maximum allowed checked
• Dropdown - options
• File Upload
• Date Picker
• Display Text - For details
• Payment Checkbox - multiple payment selection
• Payment Radio Button - single payment
• Payment text box - allow user to add dynamic amount ( this can be used for donation or other options)

Form Options

• public Link - allow to for all user or login user only
• quota - maximum allowed to form submit
• form open - close date time
• Email text after user form submit form
• allow to attachment to send in success mail

Also few other options like

• allow admin to watch all submitted form
• form fields re-orderable
• send invitation for fill form
• admin can update submitted form detail

And yes also can add multiple condition to forms

• allow to hide/show fields with different condition
