Yes, I provide unlimited MINOR revisions. This means I can change any small texts, colors, and arrangements but I cannot change the whole text content (e.g. providing a new text document to replace the original text where the design was based upon) and the whole design concept (e.g. layout changing)

Yes. If you provide different dimensions, I can provide the individual files according to each dimension but the design will be exactly the same not in different versions.

I create the illustrations myself. However, some illustrations and images I use are premium stocks that I subscribe to with premium monthly subscriptions.
