The spreadsheet must contain a realistic representation of the real-world data set. It's easiest to make a copy of the original spreadsheet, trim it down to a few rows of data, and replace any sensitive information with unique "dummy" data.
The spreadsheet must also contain the manually-entered expected results you want a formula to be returning (even if just for a few rows).
Side notes should be included, giving a concise explanation of the expected results and when you first noticed the error.

What do you mean when you say you will 'report' the problem?

What if you assess that the formula can't be fixed?
A new, blank spreadsheet has no problems. So logic would suggest that all formula errors can be fixed, even if the solution is deleting the formula and using a different approach.
Why don't you just fix the error instead of just 'reporting' it?
What can seem like a problem in a certain formula may be due to issues occuring in other formulas with cascading dependencies, or in the way an entire spreadsheet is laid out. For self-contained errors, I generally do fix the problem. For involved errors, I report the cause and recommend a solution.
