Im going to create a form with space for signatures for you. Youll be able to edit it, adding or removing questions and signature fields as many times as you want (for that, Ill give you a password). You wont have to pay for any kind of storage, and every time someone completes the form, youll receive a PDF with the responses and the signature by email. Its a simple and affordable service, and it has worked well for my clients.
