Family medicine, wound care and podiatry get denied for the same three reasons: the E/M level is not supported, modifier 25 is missing on a same-day procedure, and debridement depth does not match the note.
I code those charts so they survive payer review.
WHAT I CODE
Office and preventive E/M using the 2021 MDM rules, not defaulted to level 4.
Wound care: 97597 and 97598 debridement, 11042 to 11047 by depth and area, graft and dressing codes.
Podiatry: 11055 to 11057 callus care, 11719 to 11721 nail debridement, plus routine foot care coverage rules.
Preventive and problem-oriented visits split correctly on the same date with modifier 25.
ICD-10-CM to the specificity the note supports, including laterality and stage.

How do you decide the E/M level?
By the 2021 MDM rules: problems addressed, data reviewed and risk. I level from what the note documents, not from what was billed last time. If the note only supports a 99213, you get a 99213 and a query.
How do I send you charts?
Read-only EHR access is easiest, or send PDFs. Either way I return a coded sheet you can key in, or I enter the codes directly in your EHR if you give me access.
Do you code wound care graft applications?
Yes, including 15271 to 15278 by wound surface area and anatomic site, plus the product HCPCS. Surface area documentation is the usual failure point, so expect a query if it is missing.
How do you handle patient data and HIPAA?
Minimum necessary data only, nothing stored outside your system, no third-party AI tools, and files removed after delivery. I will sign your BAA before you send any charts.
What is your turnaround on a chart batch?
Small batches come back the next business day. For ongoing work I hold a fixed daily capacity so you know what clears each day. I will tell you up front if a deadline is not realistic.
