I provide a sheet with all bugs listed and categorized into buckets of severity, operating system. Each bug includes an annotated screenshots or a video proving the bug, and descriptive steps to reproduce
What is your price breakdown?
I charge based on the amount of work I will have to put in the project. Regardless of the complexity of the project, getting to know the project is a common denominator to all packages. Then, based on the complexity of the project I will have to invest more time, thus the price goes up

Do you find 100% of the bugs?
I go above and beyond to find bugs and provide the top experience, although keep in mind that not all the bugs can be found: 1. There are 'hidden' bugs, meaning bugs that cannot be found until other bugs will be fixed 2. I am only human (:
