I will design from scratch (not just simply convert your form unless you wish to do so) a form in Adobe Acrobat or Adobe LiveCycle which can include your logo, text fields, check-boxes, radio buttons, text boxes, digital signatures, buttons (including email submit button).
You can use your form with the free Adobe Acrobat Reader.
