Photos are usually enough for 1-2 layer boards: sharp, well-lit, straight-on shots of both sides plus close-ups of every marking, and a ruler or exact dimensions. Flatbed scans are even better. For dense or multilayer boards, physical access gives a far more reliable result.

Yes, when the nets are traceable. I identify components, map every connection, then draw a readable, properly annotated schematic rather than a raw netlist dump. Where a net cannot be confirmed, I mark it clearly in the schematic and in the delivery notes instead of guessing.
