No. These will not work as Online web forms.

I have not logo but i want order form. Can it possible?

How can i get the cards Printed?
After completing the design, I will give you the print-ready files (use the pdf format). Then you can print it.
How much time does the revisions take?
I always try to revise any project the fast as possible, but the average revision execution and deliver takes aprox 1-2 days, more or less. In any case, please be patient because quality always requires some time! :)
How many revisions do i have left?
That depends on the project and/or package purchased, but im flexible with +revisions (within reasonable), so that you can be fully happy with the final design, thats is always my main goal :)
